The integumentary system is the body's largest organ system, made up of skin, hair, nails, and glands. It protects you from the outside world, regulates temperature, senses touch, and helps make vitamin D.
The integumentary system is the body's outer covering and its largest organ system. It includes the skin, hair, nails, and a network of glands and sensory receptors. Together, these structures form a living barrier between your internal organs and the outside world.
Skin alone accounts for roughly 15% of adult body weight, making it the single largest organ. Far from being a passive wrapper, the integumentary system actively defends against injury and infection, controls body temperature, detects touch and pain, limits water loss, and helps produce vitamin D. According to NIH-published medical references, it is a "physical barrier between the external environment and the internal environment that it serves to protect and maintain."
The Three Layers of Skin
Skin is organized into three distinct layers, each with a specialized job.
Epidermis (outer layer): A tough, waterproof sheet of stratified epithelial cells. It has no blood vessels of its own and receives nutrients from the layer below. The epidermis is built from 4 to 5 sublayers (strata), with new cells forming at the bottom in the stratum basale and gradually pushing upward. By the time cells reach the surface (stratum corneum), they are flattened, dead, and filled with the protective protein keratin. Most skin fully renews its surface over a matter of weeks (roughly four weeks in young adults).
Dermis (middle layer): A thick layer of connective tissue rich in collagen and elastin, which give skin strength and stretch. The dermis houses blood vessels, nerves, hair follicles, sweat glands, and sebaceous (oil) glands.
Hypodermis (subcutaneous layer): The deepest layer, made of fat and loose connective tissue. It cushions the body, insulates against heat loss, stores energy, and anchors the skin to underlying muscle and bone.
Key Cells and Their Jobs
Several specialized cell types live in the epidermis:
Keratinocytes make up the majority of epidermal cells and produce keratin to build the barrier.
Melanocytes produce melanin, the pigment that gives skin its color and shields deeper cells from ultraviolet (UV) damage.
Langerhans cells are resident immune cells that detect invaders and help launch immune responses.
Merkel cells work with nerve endings to sense light touch.
What the Integumentary System Does
The system performs several life-supporting functions at once:
Protection. It blocks pathogens, toxins, and UV radiation while resisting physical trauma. The sebaceous glands add sebum, an oily film with antimicrobial properties.
Temperature regulation. When you are hot, eccrine sweat glands release watery sweat that cools you as it evaporates, and blood vessels widen (vasodilation) to shed heat. When you are cold, vessels narrow to conserve warmth.
Sensation. Specialized receptors detect pressure, vibration, stretch, temperature, and pain, while free nerve endings throughout the skin sense danger.
Vitamin D synthesis. When UVB light strikes the skin, it converts a cholesterol-based compound (7-dehydrocholesterol) into vitamin D3, which the liver and kidneys then activate to support calcium absorption and bone health.
Fluid balance. The barrier prevents excessive water loss and helps maintain overall homeostasis.
The Accessory Organs: Hair, Nails, and Glands
Hair grows from follicles rooted in the dermis and cycles through growth, transition, and resting phases. It helps with insulation, sensation, and protection (for example, eyelashes shielding the eyes).
Nails are hardened plates of keratin that protect fingertips and toes and improve fine touch and grip.
Sweat glands come in two types: eccrine glands (across most of the body, for cooling) and apocrine glands (concentrated in the armpits and groin, producing a thicker fluid linked to body odor when broken down by skin bacteria).
Sebaceous glands secrete oily sebum that lubricates skin and hair and limits moisture loss.
Common Skin Conditions
Skin conditions are very common. According to American Academy of Dermatology data, skin disease affects as many as one in three Americans at any given time. This section is for general education only and is not a diagnosis; talk to a clinician about any specific symptom. Common conditions include:
Acne — the AAD estimates that about 85% of people aged 12 to 24 experience at least minor acne.
Atopic dermatitis (eczema) — affects nearly 1 in 10 Americans of all ages.
Psoriasis — affects an estimated 3% of U.S. adults; it is an immune-driven condition that can cause thick, scaly patches.
Skin cancer — the most common cancer in the United States. The American Cancer Society estimated about 100,000 new invasive melanoma cases in 2024, and an estimated 5.4 million basal and squamous cell skin cancers are diagnosed each year (in roughly 3.3 million people, since some have more than one).
How to Keep Your Skin Healthy
Much skin damage is preventable. Evidence-based steps recommended by dermatology organizations include:
Protect against UV. The American Academy of Dermatology recommends a broad-spectrum, water-resistant sunscreen of SPF 30 or higher, which blocks about 97% of UVB rays. Seek shade between 10 a.m. and 2 p.m. and wear protective clothing, a wide-brimmed hat, and UV-blocking sunglasses.
Avoid tanning beds, which raise skin cancer risk.
Moisturize and cleanse gently to support the barrier.
Stay hydrated and eat a balanced diet to supply nutrients for repair.
Don't smoke, which accelerates skin aging and impairs healing.
Check your skin regularly and see a clinician about new, changing, or non-healing spots.
